Forest Transcript Request
This form should be used by any current or previously enrolled Forest
student to request a copy of an official copy of their transcript. You may print and complete this form and then mail or fax
it to the Registrar's office at the address or fax number listed on the
form. Official transcripts cost $10.00 per transcript per the Tuition
& Fee Schedule for 2008-2009. You may call 417-823-3409 if you have
any questions about submitting your request.
