Privacy Policy

HIPAA

The Robert J. Murney Clinic is in compliance with HIPAA requirements pursuant to the HIPAA Privacy and Security Act.The Clinic has instituted policy and procedures regarding the students and employees of Forest in compliance with the act. The policies and procedures are designed to enforce the federal law set forth by the HIPAA Privacy and Security Act. HIPAA training is required prior to employment or practicum training at the Murney Clinic. See the Office Manager or Clinic Director if you have not attended HIPAA orientation training. Please note, it is our ethical duty to report any known HIPAA violation regarding Murney Clinic patient information to the Clinic Privacy Officer.

The following is a brief description of procedures regarding HIPAA violations.

1 st Offense: Quiz taken on HIPAA online with a deadline for the quiz. A written warning will be issued and a meeting with a HIPAA committee member will be mandatory.

2 nd Offense: Drop a letter grade (if applicable), make a mediation plan. A copy of the mediation plan and violation will be placed in the personnel file and one copy will be sent to the Clinic Director.

3 rd Offense: Loss of Clinic privileges. A meeting with the Clinic Director and the Training Director will be mandatory.

These measures are designed to protect the clients and the future of the clinic. The responsibility is placed solely with those who take possession of the charts. Once returned to records, the Clinic assumes total responsibility for the charts and confidential information.

Based on the severity of the violation, these consequences are subject for review by the HIPAA committee to make changes to the consequences as the degree of infraction warrants.
